Terms & Conditions
The Organisation agrees to:
Provide or deliver the Supports to the 4B's Disability participant in accordance with the terms and conditions of this Agreement.
The parties acknowledge that this Agreement is bound by NDIA’s Terms of Business as in force from time to time.
The parties to this Agreement acknowledge that the Supports in this Agreement are provided to the 4B's Disability participant in the context of the NDIS, which aims to: Support the independence and social and economic participation of people with disability; and Enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.
Organisation’s Responsibilities
The Organisation agrees to:
Work with you and/or your representative (if applicable) to deliver supports that meet your needs.
Consult with the you when making decisions about how supports are provided.
Review your supports with you on a regular basis.
Communicate with you in an open, honest and timely manner.
Treat you with courtesy and respect at all times.
Acknowledge and respect your rights with regards to cultural, spiritual and gender identification diversity.
Listen to your feedback and resolve problems or issues quickly.
Give you 2 weeks’ notice if 4B's Disability needs to end your service agreement.
If applicable approve service bookings in the MyPlace Portal in a timely manner so that scheduled supports can be delivered as agreed.
Protect your and/or your representative’s privacy and confidential information in compliance with privacy law including the Privacy Act 1988 (Cth).
Deliver your supports in a manner consistent with all relevant laws, including the National Disability Insurance Scheme Act 2013 (Cth) and rules, and the Australian Consumer Law.
Keep clear and up to date records on the supports provided to you
Issue regular invoices and statements to the you, the NDIS and/or your plan manager describing the supports that we have provided to you.

You agree to:
Inform us about how you would like your services to be delivered.
Treat the 4B's Disability staff and other team members with courtesy and respect.
Talk to us if you have any concerns about the supports being provided.
Give us 48 hours’ notice if you cannot make a scheduled appointment or service.
If you are self-managed; let us know immediately if your NDIS funding is running low.
Let us know immediately if your NDIS funding is suspended or replaced by a new plan, or you stop being a participant of the scheme.
If applicable approve service bookings in the MyPlace / PACE Portal in a timely manner so that scheduled supports can be delivered as agreed. h) If applicable, provide us with evidence of your authority to act on behalf of the 4B's Disability team member if asked to do so.
Give 4B's Disability 2 weeks’ notice if you wish to end the services.
